INTRODUCTION TO UNANI

Unani-tibb or Unani Medicine also spelled Yunani Medicine (Arabic, Hindustani, Pashto and Persian) is a form of traditional medicine practiced in middle-east & south-Asian countries. It refers to a tradition of Graeco-Arabic medicine, which is based on the teachings of Greek physician Hippocrates and Roman physician Galen, and developed into an elaborate medical system in middle age era by Arabian and Persian physicians, such as Rhazes (al-Razi), Avicenna (Ibn Sena), Al-Zahrawi, and Ibn Nafis.

Unani medicine first arrived in India around 12th or 13th century with establishment of Delhi Sultanate (1206–1527) and Islamic rule over North India and subsequently flourished under Mughal Empire. An outstanding physician and scholar of Unani Medicine, Hakim Ajmal Khan (1868 – 1927) championed the cause of the Unani system in India. The Unani System of Medicine is included in the Central Council of Indian Medicine as per IMCC, Act 1970.

The Unani System of Medicine pioneered in Greece and was developed by Arabs into an elaborate medical science based on the framework of the teaching of Buqrat (Hippocrates) and Jalinoos (Galen). Since that time Unani Medicine has been known as Greco-Arab Medicine. The three fundamentals of Hippocratic Medicine were observation, experience and rational principles, which still hold valid in the field of Medicine and Science. This system is based on Hippocratic theory of four humours viz. blood, phlegm, yellow bile and black bile, and the four qualities of states of living human body like hot, cold, moist and dry.
They are represented as earth, water, fire and air, the Greek ideas were put by Arabian physician as seven principles (Umoor-e-Tabbiya) and included, element (Arkan), temperament (Mizaj), humours (Akhlat), organs (Aaza), sprit (Arwah), faculties (Quwa) and functions (Afaal). In this system, it is believed that, these principles are responsible for the body constitution and its health, as well as diseased conditions. The World Health Organization (WHO) has recognized the Unani System of Medicine (USM) as an alternative system to cater the health care needs of human population. Alternative medicine is being practiced worldwide. Unani is one of the most well-known traditional medicine systems and draws on the ancient traditional systems of medicine of China, Egypt, India, Iraq, Persia and Syria. It is also called Arab medicine.

Unani is still popular in many Arab and East Asian countries. In fact, Unani medicine and herbal products are gradually more being used in many countries where modern medicine is easily available. India has accepted it as one of the alternative health care systems and has given it official status. Practices of traditional medicine vary greatly with country and region, as they are influenced by factors such as culture, history, personal attitudes and philosophy. In many cases, the theory and application of traditional medicine are quite different from those of conventional medicine. Based on the therapies, traditional medicine can be categorized as medication and non-medication. Traditional medication involves the use of herbal medicines, animal parts and minerals. Non medication involves various techniques, primarily without the use of medication. They include, for example, acupuncture and related techniques, chiropractice, osteopathy, manual therapies, qigong, yoga, and other physical, mental, regimental, spiritual and mind-body therapies.

Unani Medicine recognizes the influence of surroundings and ecological conditions on the state of health of human beings. Apart from treating disease conditions, Unani Medicine lays great emphasis on the prevention of disease and promotion of health. In a completely healthy person Unani prescribes lifestyle, diet and environment appropriate for his or her temperament, while for those who have become vulnerable to disease, special diets, nondrug manipulations or regimens and even drugs are prescribed to maintain health and prevent disease.

The modern form of Unani medicine that we see today is a result of a long period of evolution which occurred through exchange of knowledge between diverse countries, regions and communities. The system is still increasing its dimensions and scope by incorporating the contemporary scientific knowledge and the newest of technologies. There is growing awareness among the scientific community and the general public about the intrinsic value of our traditional systems of medicine, and as a result Unani system of medicine have entered the mainstream to compliment conventional medicine.

Unani system is one of the oldest systems of medicine, based on the teachings of Hippocrates (Father of Medicine) in 460 BC, who freed medicine from the realm of superstition and magic and gave it the status of science. It is an extensive medical system that deals with various states of the body (healthy and disease). The system is integrated in nature and offers preventive, promotive, curative and rehabilitative healthcare. UNANI is derived from the word IONIAN which indicates its genesis in the present day Greece (Unan) and TIBB means medicine.

Unani system was developed in Arab and Persian lands. It was introduced by Arabs into Indian subcontinent about a millennium ago. Here it found its permanent home to see the peak of its scientific development. This system, over the centuries has incorporated so well in the Indian civilization that today it is established by Government of India as one of the Indian systems of medicine, and forms an elemental part of our national healthcare delivery system.

FUNDAMENTALS OF UNANI

In Unani system of medicine, human body is based on seven natural principles, known as “Umure Tabiya”.These are the very factors responsible for the existence of the human body and are considered responsible for the maintenance of health. The loss of any one of these components could lead even to death of the individual. These are as follows:

  • ELEMENTS (Arkan Or Anasir)
  • TEMPRAMENT (Mizaj)
  • HUMOURS (Akhlat)
  • ORGANS (Aza)
  • PNEUMA OR VITAL SPIRIT (Arwah)
  • FACULTIES OF POWER (Quwa)
  • FUNCTION (Al-Af’al)

Unani system is based on the Hippocratic theory which postulates that a perfect balance of “Arkan” (elements), “Akhlat” (humors) and “Mizaj” (temperament) keeps the body and mind healthy. The theory presupposes the presence of four humours in human body- “Dam” (blood) “Balgham” (phlegm), “Safra” (Yellow bile) and Sauda (black bile).Every individual has an inherent power of self-preservation called the “Quwwat-e-Modabbira”. Unani system of medicine aims at restoring the equilibrium of various elements and faculties of the human body.

DISEASE PREVENTION & HEALTH PROMOTION

In Unani medicine much emphasis is on the prevention of the disease and promotion of health than to cure. It also recognizes the influence of surrounding and ecological conditions on the state of health of human beings. Baqa–e-sehath (preservation of health) has been defined under Asbabe-Sittah Zaruriah.Asbabe-Sittah Zaruriah (six essential factors of health) influence each and every individual and all mental and physical diseases are due to their imbalance. For the maintenance of good health these essential factors should be balanced in terms of quality, quantity and sequence in order to sustain good health. Balanced relationship between these factors keeps the humours and temperament on the right track. The best possible way of health promotion in this holistic system of medicine is improvement of Tabiyat (Medicatrix nature or immunity).

Asbab-e-sittah zaruriyah: Asbab-e-sittah zaruriyah are six essential factors necessary for healthy life described by ancient Unani physicians. Allama Nafees defined the Asba-e-sittah zaruriyah “these are the factors which accompany a person for whole life”. Unani scholars classified these factors into six therefore these are called Asbab-e-sittah zaruriyah (six essential factors for healthy life). It includes the following six factors.

  • HAWA(Environmental Air)
  • MAKOOL WA MASHROOB(Food and Drinks)
  • HARKAT WA SUKOON BADNI(Physical Movement and Repose)
  • HARKAT WA SUKOON NAFSANI (Psychic Movement and Repose)
  • NAUM WA YAQZA(Sleep and Wakefulness)
  • EHTABAAS WA ISTAFRAAGH (Retention of Nutritive material and Regular Excretion of waste material)

DIAGNOSIS IN UNANI & MODES OF TREATMENT

In the Unani system of medicine the most important aspect forthe diagnosis of diseases are three in number,and they are Nabz (pulse), examination of baul (urine) and Baraz (stool). The surroundings and the ecological conditions are very much responsible for the state of health of a person. Much emphasis is laid on the causes for the prevention of health as (asbab sittah zaruriyah) are taken into consideration as well as on the Mizaj (temperament) of the diseased. In the Unani system of medicine there are four modes of treatment – It is special technique/physical method of treatment to improve the constitution of body by removing waste materials and improving the defense mechanism of the body to protect health. Ilaj bit tadbeer (Regimental Therapy) is moderation in Asbab sittah Zarooriyah. The procedures/Regimens (therapies) include:

  • In this therapy patient is treated with specific diets as dietitself plays an important role in human’s life.At a time it acts as a drug and very same diet can become harmfulto the same person.
  • In this therapy specific diets are administered or some alteration is done in the quality and quality of the patient’s diet.
  • This therapy aims at treating the disease with drugs of natural origin like herbs, animal origin and mineral origin. Mostly they are used as various complex compound formulations.
  • The practice of jarahat in Unani is as old as human existence. The special technique and instruments are applied in this field.

– UNANI HOSPITALS
There are 56 Unani colleges having the facility of Unani hospitals where Unani medicine is taught across the country including Odisha. As public health is State subject, setting up of Unani hospitals in the country including Jehanabad in Bihar comes under the purview respective State/UT Governments. However, under Centrally Sponsored Scheme of National AYUSH Mission (NAM), there is a provision for setting up of upto 50 bedded integrated AYUSH hospital including system of Unani and grant-in-aid is provided to State/UT Governments as per the proposals received through their State Annual Action Plans (SAAPs). Under NAM, no such proposal has been received through SAAP for setting up of Unani hospital at Jehanabad in Bihar. State/UT Governments may avail eligible financial assistance by projecting the same through State Annual Action Plan (SAAP) as per NAM guidelines.

Government of India is implementing National AYUSH Mission in the country for promotion and development of AYUSH systems including Unani system. The National AYUSH Mission inter-alia makes provision for the following: –
(i) Co-location of AYUSH facilities at Primary Health Centers (PHCs), Community Health Centers (CHCs) and Districts Hospitals (DHs) including Unani system of medicine.
(ii) Up gradation of exclusive State Government AYUSH Hospitals and Dispensaries including Unani system of medicine.
(iii) Setting up of up to 50 bedded integrated AYUSH Hospital including Unani system of medicine.
(iv) Upgradation of State Government Under-Graduate and post-Graduate Educational Institutions including Unani system of medicine.
(v) Setting up of new State Government AYUSH Educational Institutions including Unani system of medicine in the State where it is not available.
